The Jackson County Board of County Commissioners is seeking qualified professional consultants for Architectural Design & Engineering services to assist the County with the development of a complete set of design plans and bid documents for the replacement of the roof for the building formally known as “Madison’s” Restaurant, located at 2881 Madison Street, Marianna, FL 32446. It is the intent of the County to select and negotiate a contract with one (1) firm to perform these services as stated.
All responses for the project will be received at the
Jackson County Commission Room located at
2864 Madison St, Marianna, FL 32448, until
Friday, August 31st, 2023 at
2:00PM local time. At that time, or shortly thereafter, the Bids received will be publicly opened and read.
All responses will be ranked on a separate and equal basis pursuant to each firms’ qualifications and capabilities, key personnel capabilities and qualifications, client references, and previous design experience. Each category is worth 25 points with a max score possible of 100.

Addendum 1 - August 24, 2023
The purpose of this addendum is to answer bidder questions.
1. Section 5 of the proposal requirements, page 7, has a duplication: e. Sworn Statement Pursuant to
Section 287.133(3)(A), Florida Statutes on Public Entity Crimes; and: f. Public Entity Crime Statement.
Are there two separate forms? Answer: There is only one form, referenced in 5. e., and found on page
14 of the RFQ. This addendum removes item 5. “f. Public Entity Crime Statement” on page 6 of the
RFQ.
2. There are two forms included in the RFQ that are not listed in Section 5 (page 7): Program Required
Contract Clauses (pp. 19-25) and Certification Regarding Lobbying (page 27). Should these forms be
included in our submittal? Answer: Yes, the Program Required Contract Clauses found on pages 19-
25, and the Certification Regarding Lobbying found on page 27 should be included in submittals.
