BID PROPOSAL NO. 22-004
 
 
Sealed bids will be received until  2:00 P.M. CST on Monday, June 13, 2022, at Jefferson Parish Human Services Authority (JPHSA), located at 3616 South I-10 Service Rd W, Suite 200, Metairie, LA 70001, and publicly opened upon completion of administrative tasks for the following: Client Medical Supplies at Jefferson Parish Human Services Authority, 3616 S I-10 Service Rd W, Metairie, LA 70001.
 
The solicited service consists of providing medical supplies and delivery to JPHSA clients, within Jefferson Parish, on a monthly basis. A Request for Quotations packet may be obtained between 9:00 a.m. and 3:00 p.m. CST, from Tuesday May 17, 2022 until  Friday June 10, 2022 by contacting Stephanie Stephens, Procurement Analyst at [email protected].
 
All bids must be in accordance with bid documents on file with the Jefferson Parish Human Services Authority, Finance Operations, 3616 South I-10 Service Rd W, Suite 200, Metairie, LA 70001. All bidders must show the Bid Proposal Number on the outside of their bid envelope and on the bid proposal.  Late bids will not be accepted.
 
JPHSA reserves the right to accept or reject any or all proposals, in whole or in part, and waive informalities, pursuant to the law. This is an all or none bid.
