REQUEST FOR QUOTE
 
Ascension Parish Government will be accepting quotes, until November 5, 2020 @ 4:00PM, at the Ascension Parish Government Purchasing Department, 615 E. Worthey St., Gonzales, Louisiana 70737 (mailing address PO Box 2392, Gonzales, LA 70707-2392) for the following:
 
SCOPE OF WORK:
Repair/Reassemble the existing Skate Park Concrete pieces as per the drawings. The services to be provided will include the list of activities listed below. The contractor for the below list of activities shall provide all labor, equipment, materials, documentation and scheduling required to complete and document these work activities. Handling the pieces, setting and finishing will be the company’s responsibility. Housekeeping is also the contractor’s responsibility.
 
Prep site for concrete pieces to be moved.
Move concrete pieces to their location as per the drawings.
Set, level and prep the concrete pieces to be cast in concrete.
Set forms, vapor barrier, dowel rods and reinforcement to pour the connecting concrete to the concrete pieces.
Repair areas as needed due to damage during moving.

For each contract in excess of twenty-five thousand dollars per project, the public entity shall require of the contractor a bond with good, solvent, and sufficient surety in a sum of not less than fifty percent of the contract price for the payment by the contractor or subcontractor to claimants as defined in R.S. 38:2242.
 
NOTE:  Ascension Parish Government requires a one-hundred percent performance bond for all public works contracts. 
Public Works projects over $50,000.00 require the use of state licensed contractors and subcontractors. R.S. 37:2150.1. 
 
NOTE:  Ascension Parish Government requires a Contractor’s License for “all” Commercial Permits. 
 
NOTE:  Ascension Parish Government requires an Exhibit A form on file from “all” public works project vendors. 
The Parish of Ascension reserves the right to disqualify any Bid, response to a Request for Qualifications, or Request for Proposals if it is determined that the submitting business entity is not in good standing with the Louisiana Secretary of State or is not authorized to do business in the State of Louisiana.
 
The Ascension Parish Government shall not be responsible if the bidder cannot complete and submit a bid due to failure or incomplete delivery of the files submitted via the internet.
 
 
 
NOTE: Ascension Parish Government reserves the right to reject any and all quotes for just cause.
