The Jefferson Parish Hospital Service District No. 2, Parish of Jefferson, State of Louisiana d/b/a East Jefferson General Hospital (“EJGH”) is hereby soliciting the submittal of a Statement of Qualifications (Jefferson Parish Professional Services Questionnaire) from firms interested and qualified to provide professional architectural services related to the design of a third floor addition to the Cancer Treatment (Yenni) Building on our campus.

General

The scope of work associated with this project consists of the design of an infusion center and renovation of the existing 2nd floor area of the Cancer Treatment (Yenni) Building.

In order to be considered for selection: the Statement of Qualifications (Jefferson Parish Professional Services Questionnaire) must be completely filled out by the submitting firm, including applicable professional license and occupational license information and must identify all sub-consultant firms which they expect to use to provide professional services, and submit a Statement of Qualifications (Jefferson Parish Professional Services Questionnaire) for each sub-consultant firm, including applicable license information. The firm and each sub-consultant firm must also submit other information and affidavits required under Jefferson Parish Code Ordinance 2-928. (Refer to Jefferson Parish Code Ordinance, Section 2-928.)

Compensation

The Louisiana State Architectural fee schedule will be used on this project. EJGH has not accurately estimated the construction cost of this project, nor fees for professional engineering services. Fees for professional services required for this project will be established based on negotiations with East Jefferson General Hospital.

All costs associated with this project shall be subject to East Jefferson General Hospital review and approval.

Minimum Requirements for Selection

The persons or firms under consideration shall have at least one principal who has at least five (5) years experience in the field or fields of expertise required for the project.

The persons or firms under consideration shall have at least one principal who is a registered architect who shall be registered as such in Louisiana with a minimum of five (5) years experience in the discipline involved, specifically related to health care.

Evaluation Criteria

The following criteria, listed in order of importance, will be used to evaluate each firm submitting a Statement of Qualifications:

(1) Professional training and experience, both generally and in relation to the type and magnitude of work required for the particular project – 40 points
(2) Capacity for timely completion of the work – 35 points
(3) Past performance on public contracts – 20 points
(4) The size of the firm based on the number of personnel – 20 points
(5) The nature, quantity, and value of parish work previously performed and presently being performed by the person and/or firm submitting – 20 points
(6) Location of the principal office where work will be performed, with preference being given to persons or firms with offices located in Jefferson Parish – 15 points
(7) An analysis of any work by the person or firm submitting which resulted in litigation between the public entity and the person or firm performing professional services, including but not limited to ongoing litigation with a public entity or involvement in litigation with a public entity in which the public entity prevailed – 10 points
(8) Past and current professional accomplishments, for which references from clients or former clients and information gathered by inspection of current or recent projects may be considered – 10 points

To be considered for selection all firms must submit a Statement of Qualifications (Jefferson Parish Professional Services Questionnaire, 10-09-06 version) and other information required by Jefferson Parish Code Ordinance 2-928 for the contractor and any subcontractor providing professional services for the project. Please obtain the latest Jefferson Parish Professional Services Questionnaire, by calling the Parish Clerk’s Office at (504) 364-2626 or using the web address: jeffparish.net. Interested firms must submit one (1) original and four (4) copies of this Statement of Qualifications (Jefferson Parish Professional Services Questionnaire) and all additional required information and affidavits to East Jefferson General Hospital c/o Jefferson Parish Council, c/o Ms. Marylee Radtke, Vice President of Support Services & Supply Chain, 4200 Houma Blvd., Metairie, LA 70006, no later than 2:00 p.m. on August 9, 2011. No submittals will be accepted after the deadline.
