Drainage Improvements Phase 1

INTRODUCTION
The Pointe Coupee Parish Police Jury is requesting Qualification Statements from interested persons,firms, or professional organizations to provide engineering services for completion of Phase I of the
LA Hwy 81 East, Lottie, Louisiana Drainage Improvements
(HMGP #1786 -o77 -0003, FEMA - 1786- DR -LA, Project # 0046).

The procedures for the selection of this firm will be in accordance with the procurement requirements of the Pointe Coupee Parish Police Jury HMGP Procurement Policy. All responses received will be evaluated in accordance with the selection criteria and corresponding point system which is identified in the request for statements of qualifications packet. This packet also identifies
the scope of services to be performed by the selected firm.

Responses to this request must be hand delivered or mailed to the person named below at the below designated address in such a manner that it is received no later than 1o:oo a.m., on Friday,January 20, 2012. The project title, description and approximate cost are as follows:

POINTE COUPEE PARISH — LA HWY 81 EAST, LOTTlE, LOUISIANA - DRAINAGE
IMPROVEMENTS PHASE 1

The proposed project will provide additional drainage and flood protections along and crossing LA Hwy 81 through the construction of a revised drainage system.

SCOPE OF WORK

1. Phase I activities will include the following items:

Identify the watershed area and obtain detailed history documenting actual damages, perform a topographic survey, along with cross sections, to be used in developing the H & H Study and to be used for project design; identify the drainage issues and recommend drainage improvements; prepare final project plans and specifications with cost estimates;and provide project management for Phase 1 of this project.

All work as defined in Phase I must be completed no later than
October 12, 2014 as outlined in the "Approval & Funding" letter dated 11/16/11 to Pointe Coupee Parish Police Jury from GOHSEP (Governor's Office of Homeland Security and Emergency Preparedness).

Submit all work as defined in Phase I (reproducible documents) for review and approval to GOHSEP (Governor's Office of Homeland Security and Emergency Preparedness) within the aforementioned period.

2. Provide sub consultants or in -house personnel required to complete the topographic survey.

The funds approved for Phase 1 of this project are $51,000.00. The agreement will be on a lump sum,fixed price basis. Competitive proposal method will be the basis of procuring professional services required to complete Phase 1.

QUALIFICATION STATEMENTS

Submissions must address selection criteria as stated below.

All Qualification statements will include a brief history of the respondent and a resume of each person in the firm who will be assigned to the project. Describe any prior experience in which the firm has assisted a govemment entity in the Hazard Mitigation Grant Program (include the name of the government body, the type of project, the amount and the year the work was done).

SELECTION CRITERIA

All qualification statements will be scored and ranked by the Parish, with the highest rated respondent being selected to enter into contract negotiations. In the event of a tie, the respondent with the most knowledge of the specific problem associated with the proposed
project will be chosen; such knowledge must have been obtained by previous experience with either the Pointe Coupee Parish Police Jury or with similar projects regarding the affected infrastructure. Unsuccessful respondents will be notified as soon as possible.
Qualification statements will be considered by the Parish at a meeting to be held at a later date.

All responses to the Statement of Qualifications will be evaluated according to the following criteria and corresponding point system. Qualification statements will be evaluated on the basis of written materials. Sufficient information must be included in the proposals to
assure that the correct number of points is assigned. Incomplete or incorrect information may result in a lower score.

SUBMISSION REQUIREMENTS
In order to be considered, qualification statements must be received by the Police Jury prior to 10:00 a.m., on Friday, January 20, 2o12. One
(1) original and six (6) copies of the qualification statement should be submitted to Pointe Coupee Parish Police Jury, 16o East Main Street - Courthouse Annex Building (P. 0. Box 290), New Roads, LA 70760. Electronic responses sent via e-mail will not be accepted. All qualification statements should be sealed and the outside of the envelope should be marked as follows:
