Bid Law Requirement Update: Act 406
Vendors will be required to submit certain documentation with SOQ submission, i.e. affidavits, proof of insurance, etc. These requirements will be outlined specifically in the instructions of each SOQ package. Vendors should continue to carefully read and respond accordingly per the requirements of the SOQ packages. These changes are pursuant Louisiana Public Bid Law more specifically, Act 406 of the 2016 Louisiana Legislative Regular Session. ASSESSMENT SURVEY
The Parish of Jefferson, authorized by Resolution No.144736 , is hereby soliciting a Statement of Qualifications (General Professional Services Questionnaire) from persons or firms interested in conducting a senior citizen needs assessment survey for the Parish of Jefferson.
DEADLINE FOR SUBSMISSIONS: 3:30 p.m., October 25, 2024
That the Purchasing Department is hereby authorized to advertise for Statements of Qualifications from persons or firms interested in conducting a senior citizen needs assessment survey for the Parish of Jefferson.
The following criteria shall be used to evaluate the Statements of Qualification the firms/individuals submit:
1. Professional training and experience in relation to the type of work required for the services (Maximum points awarded shall be thirty (30)).
2. Size of firm considering the number of professional and support personnel required to perform the tasks. (Maximum points awarded shall be ten (10)).
3. Capacity for timely completion of the work. (Maximum points awarded shall be ten (10)).
4. Past performance by person or firm on similar projects or similar comparable projects. (Maximum points awarded shall be ten (10)).
5. Preference shall be given to persons or firms with a principal business office as follows: (1) Jefferson Parish, including municipalities located within Jefferson Parish (15 Points); (2) neighboring parishes of the Greater New Orleans Metropolitan Region, which includes Orleans, Plaquemines, St. Bernard, St. Charles and St. Tammany Parishes (twelve (12) points); (3) parishes other than the foregoing (ten (10) points); (4) outside the State of Louisiana (six (6) points). (Maximum points awarded shall be fifteen (15)).
6. Adversarial legal proceedings between the parish and the person or firm performing professional services, in which the parish prevailed or any ongoing adversarial legal proceedings between the parish and the person or firm performing professional services, excluding those instances or cases where the person or firm was added as an indispensable party, or where the person or firm participated in or assisted the public entity in prosecution of its claim. In the event that the person or firm fails to provide accurate and detailed information regarding legal proceedings with the parish, including the absence of legal proceedings, the person or firm shall be deemed unresponsive with regard to this category, and zero (0) points shall be awarded. (Maximum points awarded shall be ten (10) for the lack of any such adversarial proceedings as defined).
7. Prior successful completion of projects of the type and nature described, for which firm has provided verifiable references. (Maximum points to be awarded shall be fifteen (15)).
The person or firm submitting a Statement of Qualification (General Professional Services Questionnaire) must identify all subcontractors who will assist in providing professional services for the project, in the professional services questionnaire. Each subcontractor shall be required to submit a (General Professional Services Questionnaire) and all documents and information included in the questionnaire. (Refer to Jefferson Parish Code Ordinance, Section 2-928)
All persons or firms (including subcontractors) must submit a Statement of Qualifications (General Professional Services Questionnaire) by the deadline. The latest professional services questionnaire may be obtained by contacting the Purchasing Department at (504) 364-2678 or via the Jefferson Parish website at www.jeffparish.gov. This questionnaire can be accessed by hovering over “Business and Development” on the website and clicking on the (General Professional Services Questionnaires) option under “Doing Business in Jefferson Parish”.
Submissions will only be accepted electronically via Jefferson Parish’s e- Procurement site, Central Bidding at www.centralauctionhouse.com or www.jeffparishbids.net. Registration is required and free for Jefferson Parish vendors by accessing the following link: www.centralauctionhouse.com/registration.php.
No submittals will be accepted after the deadline.
Affidavits are not required to be submitted with the Statement of Qualifications, but shall be submitted prior to contract approval.
Proposer shall furnish the Parish with certificates of insurance evidencing Professional Liability insurance prior to contract approval.
Disputes/protests relating to the decisions by the evaluation committee or by the Jefferson Parish Council shall be brought before the 24th Judicial Court.

Is the parish looking for specific recommendations for seniors living at or below the FPL to be addressed in the response? 4. The RFP appears to only be looking for experience and qualifications, and not an approach to doing the assessment. Please confirm if you are also seeking an approach.

The Affidavit form instructions indicate the requirement for a “bar/notary number”. This is not something notaries in Michigan have. Will the form be accepted with an explanation?

We have been looking over the documents provided for the recently released Senior Citizens Needs Assessment Survey and it is still unclear exactly what services the Parish is seeking. After scanning the Statement of Qualifications, we meet the criteria described, but in our experience, senior citizens needs assessments can go in many different directions. Can the Parish provide more detail on what the desired outcomes of this study are expected to entail? For example, are the needs intended to be assessed related to any of the following: • Health and Wellness: Assessing healthcare access, chronic disease management, mental health services, mobility issues, nutrition, and overall well-being. • Housing: Understanding the availability, affordability, and safety of housing options, including independent living, assisted living, and nursing homes. • Social and Community Engagement: Examining opportunities for social interaction, volunteerism, and participation in community activities to combat isolation and loneliness. • Transportation: Evaluating access to transportation services, including public transit and specialized services for seniors with mobility limitations. • Financial Stability: Identifying financial needs, including retirement planning, access to pensions, social security, and assistance programs for low-income seniors. • Caregiving and Support Services: Understanding the availability and adequacy of caregiving services, respite care, and support for family caregivers. • Technology Access and Training: Assessing the need for digital literacy and access to technology that could improve quality of life, such as telehealth, communication tools, and entertainment. • Safety and Security: Evaluating seniors' safety in their homes and communities, including concerns about crime, elder abuse, and emergency preparedness. • Legal Assistance: Identifying the need for legal services related to wills, estate planning, power of attorney, and elder rights. • Recreation and Leisure: Assessing opportunities for recreational activities, physical fitness, and hobbies that promote active aging. 7. The below statement is in regards to the questions that were submitted for this Statement of Qualifications. This purpose of this assessment is to engage our senior citizens in an effort to determine what services they desire the Parish of Jefferson to provide to them. At the current time Jefferson Parish only provides approximately 1.5 million dollars per year to our estimated 103,000 seniors. These funds do not even cover the cost of the meals on wheels program. Other services are provided through the Jefferson Council on Aging using state and federal funds. The Parish is not trying to highlight any one particular service. We desire the consultant to provide a list of the common services provided by other large municipalities across the country. The effort is to involve in person presentations at our senior citizen centers, two public meetings and a phone survey of known home bound seniors (at least a sample size of 300). The ultimate goal of the survey is to have our seniors prioritize what services are most important to them. This information will be used to go out to our voters to solicit a greater level of funding for seniors. - The time frame to complete the survey is six months to year to be negotiated. - We do not provide budget estimates. - There is not a previous contractor in place for this service. - Refer to the Public Notice that states the SOQ requirements: - Professional training and experience in relation to the type of work required for the services - General Professional Services Questionnaire
