Sealed bids provided on the "Louisiana Uniform Public Work Bid Form Documents" will be
received by the Beauregard Parish Police Jury at its office, 201 W. 2nd Street, DeRidder, Louisiana
70634 until 2:00 P.M., Thursday, February 5, 2015.

upon deposit of $ 360.00 for each set of documents. Deposit on the first two sets are fully
refundable to all bonafide prime Bidders upon return of the documents, in good condition, no later than
ten (10) days after receipt of bids. The deposit of all other sets of documents will be refunded fifty
percent (50%) upon return of documents as stated above.
The Owner reserves the right to reject any and all quotations for just cause. In accordance with
LA R.S. 38:2212 (A) (I) (b), the provisions and requirements of this Section.
The Owner shall incur no obligation to the Contractor until the Contract Between Owner and
Contractor is fully executed.
Any person with disabilities requiring special accommodations must contact the Owner no later
than seven (7) days prior to bid opening.
The Contractor shall obtain, maintain during the life of the project and provide to the Owner
copies of the following bonds and policies which shall be obtained from surety companies which
appear on the U.S. Treasury Circular 570, licensed to do business in the State of Louisiana, in
the amounts and for the purpose stated and subject to the Owner's approval:
1. BID BOND: A "Bid Guarantee" equivalent of 5% of the bid price consisting of a firm
commitment, such as a bid bond, certified check, cashier's check, money order or other
negotiable instrument, shall accompany each bid as assurance that the bidder will, upon
acceptance of his bid, execute such contractual documents as may be required and within
the time specified.
2. PERFORMANCE BOND: A performance bond of not less than 100% of the contract price
shall be executed to assure the Contractor's faithful performance of the contract.
3. PAYMENT BOND: A payment bond of not less than 100% of the contract price shall be

executed to assure payment as required by law of all persons supplying labor and material in
the performance of the work provide by the contract.
The successful Bidder shall be required to furnish a Performance and Payment Bond written by a
company licensed to do business in Louisiana, in an amount equal to one hundred percent (100%) of
the Contract amount. Surety must be listed currently on the U.S. Department of the treasury Financial
Management Service List as approved for an amount equal to or greater than the contract amount, or
must be a Louisiana domiciled insurance company with at least an A - rating in the latest printing of the
A.M. Best's Key Rating Guide. If surety qualified by virtue of its Best's listing, the contract amount may
not exceed ten percent (10%) of policyholder's surplus as shown in the latest A. M. Best's Key Rating
Guide nor fifteen percent (15%) of policyholder's surplus as shown by surety's most recent financial
statements filed with the Louisiana Department of Insurance. The Bid Bond shall be signed by the
surety's agent or attorney-in-fact and countersigned by a person who is under contract with surety as
licensed agent in this State, and who is residing in this State.
Bids shall be accepted from Contractors who are licensed under LA RS.37:2150-2163 for the
classification of BUILDING CONSTRUCTION and show their license number on the front of the sealed
envelope in which their bid is enclosed. Bids not submitted in accordance with this requirement, shall
be rejected and shall not be read.
Bidder is required to comply with provIsions and requirements in accordance with LA, RS.
38:2212(A)(1)(c) and/or LA. R S. 39:1594(C)(2)(d). No quotations may be withdrawn for a period of
forty-five (45) days after receipt of quotations, except under the provisions of LA. RS. 38:2214.
Additional information relative to licensing may be obtained from the Louisiana State Licensing Board
for Contractors, Baton Rouge, Louisiana.
Sales and Use Tax Exemption Notice:
In accord with applicable rules adapted and promulgated by the Louisiana Department of Revenue, the
Owner shall designate the contractor and all subcontractors as its agents for the purchase and lease of
materials, supplies or equipment for the project. The contractor and all subcontractors shall accept the
agency designation. The designation and acceptance thereof shall be made on the form prescribed by
the Louisiana Department of Revenue which form shall be part of the contract between the Owner,
Beauregard Parish Police JUry, and the contractor. A copy of this form is available at the Architect's
office.
The agency relationship between the Owner and the Contractor and all subcontractors shall relieve the
contractor and subcontractor's (1) from paying any state or local sales or state or local use taxes on
materials, supplies or equipment which is affixed to and/or made a part of the real estate of the of the
project or work or which is permanently incorporated into the project or work and (2) from paying any
state or local use taxes on any materials, supplies or equipment which are leased and used exclusively
for the project or work. Accordingly, in preparing their bids and computing costs the contractor and
subcontractors shall not consider sales and or use taxes which would otherwise be due.
The Owner will furnish to the general contractor it's Certificate of Sales/Use Tax Exemption/Exclusion
on the form prescribed by the Louisiana Department of Revenue. The general contractor is required to
fill out the forms for all subcontractors and suppliers and shall send the forms to the Owner for
signature. The contractor and subcontractors shall furnish a copy of such certificate to all vendors or

suppliers of any of the materials, supplies or equipment described above.
The contractor and subcontractors shall make all purchases and leases on behalf of and as the agent
of the Beauregard Parish Police JUry.
Rules and regulations of the Louisiana Department of Revenue shall prevail over any conflicting
provisions or specifications of the contract.
A Pre-Bid Conference, with all bidders requested to attend, will be held starting at 10:00
A.M., Tuesday, January 27, 2014 in the Police Jury Meeting Room located at 201 West
Second Street, DeRidder, Louisiana and will move to the Beauregard Parish Courthouse
Building, 201 W. First Street, DeRidder, Louisiana, 70634 at 10:15 A.M.
Attendance at the Pre-Bid Conference is optional, but highly encouraged.
